Household Information Form

All students in our district will be receiving free healthy breakfast/lunch for the second year.

In place of the Free and Reduced Application we still need your household to fill out and sign the Household Information Form.

Forms will be going home today with the youngest student enrolled at Maples School. Only one form is needed per household
