Michigan Healthy Smile forms will be sent home today with your child. Forms are due Thursday, October 28.
Mobile dentist will service students at Maples School on Wednesday, November 3, Thursday, November 4 and Friday, November 5.
**A state licensed dentist will check your child’s mouth and teeth as well as provide cleaning, x-rays as necessary, fluoride treatment and apply sealants, as necessary. A dental report card will be sent home with your child.**
**Students without insurance will still be seen free of charge. Please make sure to fill out the form and send it back to school with your child.**
