REMINDER:
Michigan Healthy Smile forms are due Monday, November 4, 2019.
A state licensed dentist will regularly check your child’s mouth and teeth as well as provide cleaning, x-rays as necessary, fluoride treatment and apply sealants, as needed. Additional care, such as fillings may also be provided. A dental report will be sent home with your child.
All students who turn in a form will be seen regardless of health insurance status!!
Mobile dentist will be at Maples School the week of November 11-November 15 .
